We ask that you complete the Patient Portal at least 48 hours before your scheduled appointment.  To do so, please click on the link below. It will open up the registration page for the portal. Please enter your email address and the password provided to you to gain access to the portal.

**The portal should be done on a desktop or laptop Mac or a PC only.**

Those it is accessible on mobile devices, not all features are maximized for use on mobile devices and you will not be able to complete the portal.  We are working on a large upgrade that will allow the portal to be completed on mobile devices as well as allow you to enter your insurance information, upload before pictures and more.  It will be rolled out in late summer 2020.

Please pay special attention to the “Additional Complaints” section of the medical history and make sure that check “No Issues” for any system that you are not having problems with.  If you have difficultly completing the portal or need additional assistance, please contact me at trevor.hoppers@weightwise.com.
